\tDear Sir, Subject:  My ex-employer Provident Fund Form has not been attested. This is to inform you that, I was employed with M\/s “CVK Mind Source Consulting Services Private Limited” bearing PF A\/C No KN\/BN\/41509\/358 and left the said company on 30\/04\/2010.<\/p>\n

\tI am continuously trying to withdraw my P.F amount from last three year but not getting the response from the ex-employer. In this three year I had got dangerous diseases- cancer and brain ham rage (Right hemisphere disability (paralyzed) – since 16.07.2010 – during Chemotherapy treatment).<\/p>\n

\tSo, Documents attested by Authorized officer and sent by speed post (EU875207080IN) to "Office Of Regional P.F Commissioner, Bhavishyanidhi Bhavan, No. 13, Raja Ram Mohan Roy Road,  Bangalore – 560 025 (KN)"  on 22\/04\/2013.<\/p>\n

\tRequesting you please see this matter on high priority basis and  please revert back to me with complaint number.<\/p>\n

\tI am sent the complete documents for your reference i.e. offer letter, experience letter, salary slip, Resignation Letter, SALARY CERTIFICATE, salary structure, bank statement , e-mail’s, paralysis certificate, medical certificate, Employee details, Employer details and complaint application.<\/p>\n
